Brookside Consultants Inc. partners with an experienced panel of physical therapists to ensure that a timely, objective and accurate functional assessment is performed and reported.

FCE requests are accepted via phone, fax, internet or e-mail. Our Service Representative will also come to you and complete the referral sheet as well as copying the medical records, if you so desire.

If a script is needed, our on-site Registered Nurse will contact the Attending Physician’s office to secure the script prior to scheduling the FCE.

Once the appointment is scheduled and entered into our system you will immediately receive an appointment confirmation letter and an appointment letter that was sent to the patient. A copy of these letters will be sent to you via e-mail as a Microsoft Word attachment (e-mail address and Microsoft Word required).

Once the completed FCE is received it will be distributed to all parties of interest within 24 hours of receipt. Also, we will fax a copy of the report summary to the adjuster promptly upon receipt.
